Entity names not properly filling the preferred_label fields on import

I ran an import on command line for person names data, but the last name and first name got filled redundantly. The data source and the mapping look fine to me...

Below is what the log file says, but I don't know why it thinks the forename is empty.

    You need to set the group for these three mappings:
    ca_entities.preferred_labels.displayname, ca_entities.preferred_labels.surname, ca_entities.preferred_labels.forename 
    to the same value. That tells the importer to consider the three mappings as a unit. Right now it's interpreting them as three separate labels.
