Thumbnails in displays

I've learned to do so much in CA, but I can't seem to figure this one out: No matter how I define a display, the resulting PDF includes a thumbnail. I've read all the docs and searched the forum. I see in the docs that a display can be full, list, or thumbnail. I've never noticed a place to force it to a list rather than a thumbnail type. I don't want the thumbnail. See attached screen shots. Using CA1.5

It appears I may need to change the checklist print template to remove the thumbnail (which should not have been there from the start).
